Reimagining Your Career in Hospice

Caring for others is more than what you do - it's who you are. At AccentCare, you'll join a purpose-driven, collaborative culture that sets the standard for excellence and gives you the trust and tools to do your best work. You'll belong to a team that cares deeply for patients and each other; a team committed to consistently providing exceptional care.

We're proud to be named one of America's Greatest Workplaces 2025 by Newsweek - a reflection of our shared commitment to excellence, integrity and compassion as we shape the future of aging in place. When you thrive, so does the community of care we're building together.

Be the Best Hospice LVN/LPN You Can Be

If you meet these qualifications, we would love to meet you:

  • Must have a current Licensed Practical Nurse (LPN) license in the state of practice; associate degree preferred. CPR certification required in Georgia and preferred elsewhere
  • Must meet all state and regulatory requirements where care is provided. One year of experience in a licensed healthcare setting is required (preferred in California), ideally in hospice, home health, geriatrics, or medical-surgical care
  • Must have a valid driver's license, reliable vehicle, and state-required liability auto insurance
  • Must be committed to the organization's values, mission, and philosophy of care, serving as a role model for team members
  • Physical demands include frequent walking, sitting, stooping, driving, typing, and lifting or moving up to 50 lbs; must assist with patient handling up to 200 lbs with help
  • Must be comfortable working in a variety of conditions, including occasional exposure to outdoor weather, noise, fumes, chemicals, and uneven surfaces

Responsibilities:

  • Provide direct nursing care under the guidance of a Hospice RN to support patient comfort, health, and dignity across pre-hospice, continuous care, and in-home respite services
  • Collaborate with the interdisciplinary team (IDG), patients, families, and physicians to assess needs, coordinate care, and support holistic well-being
  • Maintain timely, accurate documentation and communicate any changes or concerns to the clinical team to ensure responsive, high-quality care
  • Educate patients and caregivers in alignment with the plan of care, promoting understanding, confidence, and comfort throughout the care journey
  • Participate in on-call rotations, scheduled visits, IDG meetings, and family conferences, contributing to seamless care coordination
  • Remain current with hospice protocols, licensure requirements, and organizational standards through ongoing training, committee participation, and interdisciplinary collaboration

Our Investment in You

Caring for others starts with caring for you. We're committed to fostering a purpose-driven workplace where you feel supported, and that means prioritizing your physical, financial and mental well-being. Our benefits include:

  • Medical, dental, and vision coverage
  • Paid time off and paid holidays
  • Professional development opportunities
  • Company-matching 401(k)
  • Flexible spending and health savings accounts
  • Wellness offerings such as an employee assistance program, pet insurance, and access to Calm, a meditation, sleep, and relaxation app
  • Programs to celebrate achievements, milestones, and fellow employees
  • Company store credit for your first AccentCare-branded scrubs for patient-facing employees
  • And more!
