Key Responsibilities:

• Conduct comprehensive client assessments to create personalized care plans
• Administer medications, treatments, and nursing procedures as required
• Monitor clients' health status (vital signs, progress) and adjust care plans as needed
• Educate clients & families on disease management, medication use, and home safety
• Perform advanced nursing procedures including wound care, IV therapy, and catheter care
• Document all care provided in compliance with regulatory guidelines
• Ensure compliance with state and federal healthcare regulations
• Provide emotional support and guidance to clients and their families
• Stay up-to-date with nursing best practices and participate in professional development

What We’re Looking For:

• Active RN License in the state of New York
• 1-2 years of nursing experience (home health, geriatrics, or similar preferred)
• Strong clinical skills and ability to perform nursing procedures independently
• Excellent communication skills and the ability to build rapport with clients and families
• Self-motivated and organized, with the ability to work independently in a home care setting
• Reliable transportation and a valid driver’s license
• CPR and BLS certification (required)
• Experience with electronic health records (EHR) systems (preferred)
