Job Title: Hospice Nurse (RN or LPN) Location: Hybrid - Wellspring Hospice Frankenmuth Reports to: Hospice Clinical Manager Position Type: Per Diem | Hourly Accepting applications through November 25th, 2025.

Wellspring Lutheran Services is a statewide nonprofit dedicated to serving families throughout Michigan. We are currently seeking a Per Diem Hospice Nurse (RN or LPN) to join our Hospice team serving Saginaw, Frankenmuth and surrounding areas. This role provides in-home and facility hospice care that ensures comfort, symptom management and compassionate support for patients and their families.

Responsibilities

• Obtain and utilize all necessary information to function effectively in hospice settings
• Provide professional nursing care and comfort using the nursing process
• Assess and evaluate patient and family status, including:
• Performing initial patient assessments (RN)
• Reviewing and implementing the Plan of Care for pain and symptom relief
• Regularly re-evaluating patient and family needs to ensure comfort and support
• Revising service delivery plans as necessary
• Participating in interdisciplinary group meetings
• Administer medications and treatments as prescribed
• Apply appropriate preventative, therapeutic, and rehabilitative nursing procedures and techniques
• Maintain communication and coordination with physicians, supervisory personnel, and other disciplines regarding patient care
• Complete, maintain, and submit accurate clinical notes according to procedure
• Supervise hospice aides every 2 weeks (RN)

Qualifications

• Current and in good standing Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
• Successful completion of an accredited nursing program
• At least 1 year of nursing experience within hospice (preferred); 1 year of home health, medical, surgical or critical care nursing experience acceptable
• Compassionate, reliable, and committed to patient and family care

Schedule and Compensation

• Employment type: Per diem, as needed
• Weekends: 1 weekend required per month; ability to pick up extra as desired
• Hybrid Remote: Home-based with travel to patients in-home and in skilled nursing facilities
• On-call: scheduled participation required
• Pay Rate: $27.00+/hr (LPN) or $38.00+/hr (RN) based on experience and licensure
• Mileage reimbursement provided
